Insurance
Depending on your insurance plan, the coverage for your upcoming lab draw may vary. Your insurance company may cover the full amount, leaving no cost to you. However, in some cases, they may only cover a percentage of the cost.
If there is any remaining balance not covered by your insurance plan, you will be invoiced for the amount or required to pay in full before your scheduled appointment.

Step Two:
Upload Lab Order and Insurance card
Step Three:
Once approved, someone will schedule a time and date that works for you.
Step Four:
Once scheduled, you choose if you want a mobile draw or an on-site collection.

Please download the documents & sign. This is a one-time use only. You will not have to do this again if you keep using MD Lab Solutions for your lab collections.
